Submitting Forms
If you require a form to be completed by your family doctor please see below for more details
- All forms require a minimum of 2-3 weeks for completion from the day your doctor receives the form. Wait times can vary depending on the type of form. We ask that you please refrain from calling the office regarding the status of your form, as we are unable to provide an exact completion date ahead of time.
- If your form is work related, it is your responsibility to inform your employer of expected delays in processing time.
- Some forms may require an appointment to complete, in which case you will be contacted to book an appointment with your doctor.
- Please note, all forms are subject to a possible fee at the discretion of your doctor.
- Please inform the office at the same time of submission if your form has a specified due date.
