It is very important that you have a valid Ontario health card and that you present it to reception at each visit.
Our office health card policy is as follows:
We no longer accept invalid or expired Ontario health cards. Patients who arrive for an appointment with an invalid or expired health card will be charged a fee of $30.00 for each appointment until health card has been updated.
This fee can be reimbursed if a valid health card is presented within 60 days of the appointment. Please be sure to keep your receipt: reimbursement CAN NOT be processed without the original receipt.

It is very important that you notify us with any address/phone number changes in case we are needing to reach you.
Please also ensure that you update this information with Service Ontario.
Certain services provided by our physicians are not covered by OHIP. These services include (but are not limited to):
If you are needing any of these services you will be charged a fee.
Ask your doctor about our annual extended coverage.
If you are unable to come in for an appointment to have your prescription renewed you can have your pharmacy fax a renewal request to your doctor. Please be aware that there may be a fee of $15 charged for renewals without an appointment.
Please allow a minimum of 48 business hours for processing of ALL prescription renewal requests.
PLEASE NOTE: It is always best to schedule an appointment so that your doctor can assess if the medication/dose is still suitable for you.
If your doctor has made a referral to a specialist for you the office will contact you once we have an appointment date/time. Depending on the specialist it may be weeks, or sometimes months, before we are given any appointment information.
PLEASE NOTE: It is your responsibility to reschedule or cancel any specialist appointments. Doing so in a reasonable amount of time allows them to use that appointment slot for someone else.
If you fail to show up for your appointment you will likely be charged a fee by the specialists office.
