Clinic Policies

Health Cards

It is very important that you have a valid health card and that you present it to reception at each visit.

If your health card has expired you do not have valid health coverage and may be charged a fee for your appointment at our clinic, the after hours clinic, specialists offices, hospitals and other healthcare facilities.  To see our full office policy regarding Health Cards please click HERE.

Patient Contact Information

It is very important that you notify us with any address/phone number changes in case we are needing to reach you.

Please also ensure that you update this information with Service Ontario.

Uninsured Services

Certain services provided by our physicians are not covered by OHIP.  These services include (but are not limited to):

  • Sick Notes
  • Prescription renewals without an appointment
  • Various Forms (Insurance, Ministry of Health, etc.)
  • Drivers Physicals
  • Transfer of Medical Records


If you are needing any of these services you will be charged a fee.


Ask your doctor about our annual extended coverage.

Prescription Renewals

If you are unable to come in for an appointment to have your prescription renewed you can have your pharmacy fax a renewal request to your doctor.  Please be aware that there may be a fee of $15 charged for renewals without an appointment.

Please allow a minimum of 48 business hours for processing of ALL prescription renewal requests.


PLEASE NOTE:  It is always best to schedule an appointment so that your doctor can assess if the medication/dose is still suitable for you.

Specialist Appointments

If your doctor has made a referral to a specialist for you the office will contact you once we have an appointment date/time.  Depending on the specialist it may be weeks, or sometimes months, before we are given any appointment information.


PLEASE NOTE: It is your responsibility to reschedule or cancel any specialist appointments.  Doing so in a reasonable amount of time allows them to use that appointment slot for someone else.

If you fail to show up for your appointment you will likely be charged a fee by the specialists office.  

Travel Medicine

The doctors at Norfolk Family Medical do not offer travel advice.

Winter Advisory/ Inclement Weather

In the event of inclement weather  

our office follows the guidelines of 

The University of Guelph. 

If the University of Guelph 

is closed, our office will also be closed.

If you have an in-person appointment scheduled, it will be switched to a telephone appointment; unless you are notified otherwise by office staff. 

All other appointments (virtual video or telephone) will continue as booked.
